What type of behavior is it?
Minor behavior managed by teacher
Major behavior managed by Principal
Did the behaviour resolve? If yes, continue to acknowledge student for making positive choices.
If not, do the following.
If the similar behaviour continues more than 3 times then it becomes a Major behaviour.
3. Record incident on Assembly and monitor the frequency, time, location, and possible triggers.
Share information with Principal and contact whanau.
4. Create an Individualised Behaviour Plan with Team Leader. Involve student and whanau.
Follow through with Plan for a sustained period of time.
5. Continue to keep Team/Senior Leaders informed and keep an accurate record on Assembly.
Incidents and records are recorded on the day of the incident/occurrence on Assembly. Any follow up notes regarding outcomes or consequences are added within the week. When meeting with students and whanau in a conference situation notes are to be recorded in the classroom orange book with a carbon copy being sent home with whanau.

► Report to the BOT and MOE
